General information

Our public website provides information about our services, staff opportunities, and how to contact the office. Please do not include protected health information in public contact fields or ordinary email.

Information you submit

When our two public forms are activated, the general inquiry form collects your name, address, email, phone number, topic, general message, and any optional nonclinical attachment. The separate careers application may collect first and last name, email, phone and cell numbers, position of interest, an optional message, and an optional résumé. We use the information to respond to inquiries or consider applicants for recruitment. If the forms are activated, submissions and optional attachments are processed by our website security and transactional-email providers (Cloudflare and Resend) and delivered to ACT’s Microsoft 365 office mailbox. The website does not maintain a separate form-submission database. These providers may retain operational logs and email data under their service policies. Do not send medical records, Social Security numbers, or other highly sensitive information through either form.

Website technical data

Website hosting and security providers may process essential technical data such as IP address, user agent, access times and security signals to deliver the website and protect it from abuse. We do not presently claim to offer a patient portal on this site.

Handling confidential health information

This site is not a secure patient portal. Clinical referrals and records must be provided through an approved secure intake process arranged with our office. Patients may receive separate HIPAA Notice of Privacy Practices documents describing the privacy of their health information.
